Introduction Three simple equations based on pretreatment prostate-specific antigen ( PSA T stage, and Gleason score can be used to predict path-ologic stage and the risk of treatment failure in patients with localized prostate cancer, reported Mack Roach III, MD, at the First Sonoma Conference on Prostate Cancer. Dr. Roach is Associate Professor, Radiation and Medical Oncology, at the University of California, San Francisco. The equations are used to calculate risk of biochemical failure—rising PSA—following surgery or radiotherapy. Dr. Roach reviewed studies showing the close correlation between rising PSA and declining survival “to reinforce the point that PSA is likely to be a valid end point—surrogate end point, probably—for survival.” The equation for extracapsular extension is based on pretreatment PSA and Gleason score ( GS). The equations for lymph node involvement and non-organ confined disease add T stage to these factors ( Table 1). Risk for Extracapsular Extension  The original equation predicted the risk for extracapsular extension ( ECE). That equation is : ECE risk = (1.5 x PSA) + [( GS - 3) × 10]. For use in these equations, the nine possible Gleason scores are grouped into five major scoring categories: Gleason scores 5, 6, and 7 remain discrete categories, and scores of 4 or less and 8 or higher are lumped into a single category. Applying the ECE risk to about 374 patients who underwent radical prostatectomy at UCSF, produced “an excellent correlation,” Dr. Roach said. He next looked at how the equation would correlate with outcome of patients treated with radiotherapy. He found that the equation identified patients with relatively low, intermediate, and high risk of progression. “ In general, it separated these patients out better than using other parameters, like T stage and Gleason score, although it’s unclear how much the equation adds over PSA.

A wave of outrage followed Ubisoft’s latest ‘security’ advancements. Some of their latest releases insist on an internet connection to start the game. Of course, if Ubisoft’s servers are down, or if you simply aren’t connected to the internet at the moment, you won’t be able to play. Why Honest Consumers Will Want to Know Play Games Without CD Whatever your opinion on piracy, the facts are simple. Copy protection isn’t hurting them. It takes less than a minute to work around it. It’s the honest consumer that’s affected. Then, instead of simply recognizing it as a necessary evil, it’s your every right to do something about it. Run the auto-patch program. If you installed Map Send in the default location, the program will find and highlight the Map Send.exe file exactly as shown. Hey folks, I've read quite a few threads of folks having trouble with their use of Original CD Versions of Kot OR 1 & 2.  In order to simplify things, I've decided to create this handy - all-in-one thread of how to install Kot OR if you're using Vista or Windows 7. This is should NOT be used if you're dealing with the Steam version of Kot OR or Ko TOR2.   This thread is mostly a cut and paste job put together from sources from both here and at Lucas Forums. This is what I personally use to install the games when I'm doing a fresh install. Credit for these instructions should go to Renegade Sniper7, Astrotoy, Q, and alinadem. Blame to should go to me in case I've royally screwed things up in putting this thread together. This will be a locked thread as I've not the time to respond to the multitude of question this thread should create. Definitive Order of Installation and Fixes for the Original CD Versions of Kot OR 1 & 2 - NOT the Collection DVDs! ( Also Works for Windows 7 Before we begin. There are many reasons one may want to play the original unmodified IL2. And also many reasons you may want to protect your installation DVD. Especially if you are stuck donw here in South Africa where you will NOT get a replacement if your DVD drive eats it.( And apart from that, I am of the school of thought that anti-piracy software, measures and restrictions do 0% to stop piracy, and only punishes legitmate users) So, here is a exe file that replaces the installation stock one, and makes you able to run the unmodded game without the DVD. It works for all DVD versions as far as I know, at least 4.05 and later. The modded game of course has always run without DVD, but that is a whole different story, isnt it? So, this is not a mod, as much as a crack. Use it like an adult, use it at your own risk. I have been using it for years. This no cd crack works with 4.09m. Download.
